Membership includes one Comprehensive Annual Wellness Assessment, comprehensive laboratory panel, and a personalized wellness plan annually. Adult and dependent sick visits are available for a flat rate of $35 per visit, payable at the time of service. Annual enrollment fee. Memberships are HSA/FSA eligible. We do not bill insurance.
